Exciting, innovative nursing opportunity to join a new team providing support to patients through web based technology!
Reporting to the Program Manager, HealthForward, the Nurse Case Manager is responsible for supporting patients throughout their chronic disease journey across a number of therapeutic areas and drugs by delivering personalized care. The Nurse Case Manager will facilitate  coordination of care and services between the patients extended care team, service providers, and patient support programs.      
  • The management of patients according to personalized care plan, providing motivational encouragement to help patients stay adherent to their care plan and treatment goals, attempting to get them back on track should they become non-adherent
  • Telephone/web support
  • Implementing call-center program including the provision of patient and healthcare professional bodies with telephone/web support. Current assistance lines focus on being the first point of contact for patients, their family and loved ones, the extended care team, and other users of the platform
  • Conduct scripted and unscripted health coaching conversations with patients taking into account their psychological profile. During those, provide motivational encouragement and help patients stay adherent to their care plan and treatment goals, attempting to get them back on track should they become non-adherent
  •  Provide chronic disease management (including compliance, utilization, educational information on specific products/therapeutic areas/devices/delivery methods/technical support)
  • Administer and adhere to case-management protocols, including symptomatic and preventative care interventions to help improve the condition or wellbeing of a patient, within the clinical governance defined in the care plan
  • Supporting the patient in-between visits through clinical monitoring, data collection, on-going patient follow-up, compliance, therapy disruption/discontinuation monitoring, adverse event reporting and liaison with primary healthcare providers
  • Document each activity as required
  •  Participate and attend on-going refresher training and specialized training courses as deemed appropriate by the manager
  • Provide feedback regarding current protocols, database, processes, inter-departmental relationships
  • Clear communication to manager, and program trained colleagues regarding the management of assigned patients during foreseen absences
  • With the managers, participate in the training process of new caseworkers, including but not limited to shadowing & call monitoring
  • Adherence to the general HealthForward Inc. processes, as well as program specific processes


Job Requirements

  • Registered Nurse License for province of employment, or Registered Practical Nurse License for province of employment
  • Bilingualism (French and English) strongly preferred;
  • Knowledge and experience in areas of Multiple Sclerosis and Rheumatoid Arthritis required
  • 1 to 3 years of nursing experience required
  • Strong patient focus
  • Professional/courteous/efficient telephone manner
  • Strong ability to multi-task
  • Must be detailed-oriented and organized
  • Ability to work under pressure
  • Ability to compassionately identify and convey disease state and product delivery information to patients preferred
  • Working knowledge of Word, Excel and Access
  • Working knowledge of advanced internet technology
  • Experience in customer service an asset
